Supplemental Application Instructions and Procedures

Fall Entry 2020

The accuracy, completeness, and neatness of your application are determining factors in our admission decision. Failure to submit a complete or accurate application may result in disqualification for admission or dismissal from the program. The Admissions Committee will not evaluate an incomplete application.

Two-Part Application Process

Both the PTCAS Application and the Pacific University Supplemental Application, with supporting documents, are required to apply to the program:

PTCAS application + Pacific University Supplemental Application = COMPLETE APPLICATION

Both applications must be submitted online by the October 1, 2019 date. This includes the PTCAS Application and the online Pacific University Supplemental application. Please note: it can take 4-6 weeks to receive supporting documents and process an application. Submit your letters of reference and transcripts to PTCAS early! Pacific University assumes no responsibility for materials not received by the deadline from PTCAS.

Pacific University Supplemental Application

  1. Application Fee -- A non-refundable application fee of $40 (US) must be paid at the time you submit the supplemental application.
  2. Evaluation Forms/References -- All reference forms should be submitted directly to PTCAS. PTCAS will accept up to 4 reference forms, however, Pacific University only requires 2 references. If you submit more than the required 2 references, please specify in the designated supplemental application area which (2) references you would like Pacific to review. At least one reference must be from a current, practicing, licensed physical therapist with whom you have worked.
  3. Essays -- Respond carefully to the supplemental essays as instructed in the application. Please note the word limits for the essay responses. Through the essay questions, we are trying to learn something about you as an individual, as well as your interest in the field of physical therapy. Do not repeat or rephrase your Personal Statement from the PTCAS application.
    • Re-applicants - Please note the additional required essay question for re-applicants to the Pacific University Physical Therapy program.
    • Optional - The two optional essays are an opportunity to provide any additional information about yourself for consideration, including explanations for any prerequisite or coursework issues.
  4. Prerequisite Worksheet -- Enter coursework you have completed and plan to complete by the start of the program in August 2020. You must have a plan to complete all prerequisite coursework listed on the worksheet. Science and statistic coursework must be completed within 7 years of matriculation.

Pacific University Supplemental Application

Re-Applicants: You must fully complete both the PTCAS Application and the Pacific University Supplemental Application. All official transcripts should be sent to PTCAS. Please note the required re-applicant essay question in the Pacific University application.

Interviews: On-campus interviews are by invitation only. Interviews are conducted in January/February and are full day events organized specifically for physical therapy candidates. Invited applicants must participate in the interview event to be considered for admission.
