- Application for Admission. Complete and submit the on-line application.
- Student Recommendation. Complete the information above the black line and have the form completed and returned by the appropriate person. (View and Print Recommendation Form)
- High School Transcript. Request that your high school forward a final, official transcript directly to the Office of Student Affairs at Baker University School of Nursing.
- College Transcript(s). Request that final, official college transcripts from any college(s) attended be sent directly from the Registrar of the college to the Office of Student Affairs at Baker University School of Nursing.
Students with college work in progress may be admitted based on an official but incomplete transcript, but an official transcript must be submitted upon completion of the hours in progress. Such admission decisions are subject to review if the final transcript reflects a change in academic standing.

Other Required Documents and deposits
After a student has been notified of acceptance, the Office of Student Affairs will provide a Health History Form to be returned prior to enrollment.
Admission Requirements
To be eligible for admission to the School of Nursing as a transfer student, applicants must have attained a 2.7 college grade point average in the required prerequisite courses.
Any applicant who does not meet the criteria above will be reviewed by the Student Affairs Committee at Baker University School of Nursing.
Notification
All admission decisions are made on a rolling basis with notifications usually sent two weeks after the receipt of all required documents and pre-admission interview.
Please be advised that nursing licenses may be denied or licenses disciplined by the Kansas State Board of Nursing (KSBN). Possible grounds for such action may include being guilty of any felony or being guilty of a misdemeanor that involves an illegal drug offense if the KSBN determines that such person has not been sufficiently rehabilitated to warrant the public trust. Also considered are patterns of practice and other behaviors exhibiting an apparent incapacity to practice nursing. All criminal history must be revealed to and will be evaluated by the KSBN BEFORE licensure is granted or denied. (Kansas Nurse Practice Act Law and Administrative Regulations)
