Getting Started

In order to submit the Acknowledgement form or the Disbursement Request form, you will need an ApplyWeb account.

You may already have an ApplyWeb account if you have applied to college through CollegeNET. Please note that you cannot use your account to complete these forms.

If you already have an ApplyWeb account, you do not need to create a new account.

Create a new account or log in using your email address.

Apply Online

Acknowledge award from current or most recent calendar quarter and if ready, request disbursement. You must submit this form within 30 days of the end of the calendar quarter or you will forfeit your award(s).

The calendar quarters and corresponding Disbursement Request Periods are:

  • January 1 - March 31 (Disbursement Request Period: April 1-30)
  • April 1 - June 30 (Disbursement Request Period: July 1-30)
  • July 1 - September 30 (Disbursement Request Period: October 1-30)
  • October 1 - December 31 (Disbursement Request Period: January 1-30)

Student Loan Repayment: Download a blank W-9 Form

Be sure to read all instructions within the form (or on the FAQ) about student loan repayment, as school loan relief is a reportable tax event.
