Required materials to complete your application package
Print this page to use as a checklist to ensure that your application package is complete.
Check off each item as you put it in one envelope.
The MITE2S office will not accept an incomplete application package.

Official high school transcript for Grades 9 & 10 (if in a language other than American English, provide translation)

11th Grade 1st semester grades or copy of report card (if in a language other than American English, provide translation)

Copy of PSAT or SAT 1 or ACT scores

Guidance Counselor Evaluation on school stationery, sealed and signed

Math Teacher Evaluation on school stationery, sealed and signed

Science Teacher Evaluation on school stationery, sealed and signed

Social Security number or copy of Social Security Card

Two wallet-size (or passport size) photographs; write your full name on the back of each photo

If U.S. citizen, copy of birth certificate OR if Permanent Resident, copy of Permanent Resident "Green" Card

If Native American, copy of Tribal Card
Four essays
One essay per sheet of paper. Each essay should be 300 words or less. Type or
print using black ink (no pencil) on white paper.

Essay 1: What are your career goals? How might attending MITE2S support these
goals?

Essay 2: Which one of your extracurricular or volunteer activities is most
important to you and why?

Essay 3: How has your racial or ethnic identity affected your personal
development? How has it influenced your aspirations?

Essay 4: Please describe a personal, social, or academic challenge you have
faced and overcome.
Make sure

Your name is printed clearly on every item in your application package.

All staples have been removed and your application package kept together with one paper clip.
