  1. Application Fee: The application fee is $0 if the application is submitted by the preferred deadline date. If you submit after the preferred deadline date, the $40 application fee is required and can be submitted electronically when you submit the application online.
  2. Resume: Submit a resume/CV with this application listing previous education, work, and other relevant experiences, including:
    1. Your role in any human service work you have done, the kinds of clients served, the kind and amounts of supervision you received, and the discipline of the supervisor.
    2. Special skills relevant to mental health, psychology, or human resources work.
    3. Any publications or presentations.
  3. GPA: Calculate your GPA using all graded courses you have taken for your undergraduate studies, last 60 hours, and graduate studies on the GPA Calculation Worksheet in the online application. Failure to calculate your GPA may result in an incomplete application.
  4. Transcripts:
    • Applicants are required to upload unofficial transcripts from each college, university, or professional program attended, including institutions where courses were completed but no degree was awarded. These must be uploaded in the Academic Information section of the application. Unofficial transcripts must clearly list your name, the institution’s name, all completed coursework, and grades received.
      • If you are admitted, official final transcripts, including verification of degree conferral, will be required prior to enrollment. We will contact you with instructions for submitting official transcripts, so you do not need to take any action on this now.
    • International Transcripts: If you attended a college or university outside the U.S. or Canada, you are required to submit an official, course-by-course credential evaluation of your academic records. This evaluation must be completed by a recognized third-party organization, such as World Education Services (WES). The credential evaluation must be received by the final application deadline in order for your application to be considered.
    • For more information, visit the WES website at http://www.wes.org.
  5. References: We require a completed evaluation form with an attached letter of recommendation from a minimum of two (2) individuals who have worked closely with you. Instructors from prior academic programs and supervisors in the field are the preferred source of references. Please follow the prompts in the reference section of the application to complete this requirement. Each recommender listed will receive an email with instructions on how to electronically submit an evaluation on your behalf. An attached letter to the evaluation form is a requirement. It is very important that you inform your recommenders of the necessity to download and attach a letter to their evaluation form.
  6. English Proficiency: All applicants whose first language is not English are required to demonstrate English proficiency. All test scores (TOEFL or IELTS) must be received by our final application deadline to ensure consideration of your application.
  7. GRE: GRE Requirement - The GRE is optional for our PhD candidates. We do not require the GRE in order to apply. If you have scores and would like them to be considered as part of your application profile, please submit them with your application. Please note that by not submitting scores, you are in no way hurting or negatively affecting your application.
